0103-REPORTING CANCER SURVIVAL

DEFINITION OF SURVIVAL DURATION

 

               Time from diagnosis to death

 

               Time from start of treatment to death

 

MEASURES OF SURVIVAL

 

               6-month survival:  % of patients alive after 6 months

 

               1-year survival: % of patients alive after 12 months

 

               5-year survival: % of patients alive after 60 months

 

               Median survival: Time until 50% of the patients have died

 

PURPOSE OF SURVIVAL ANALYSIS

 

               Overall evaluation of cancer management programs: screening, diagnosis, treatment

 

               Prognosis

 

               Planning

 


 CNF: THE PRESENT STATUS

 

               Alive means known to be alive on data of notification. If last known to be alive at an earlier date, please write that date

 

               Dead means known to be dead on the date of notification. The information could be from clinical records, death certificates etc

 

               Date of death is recorded if known. You may record month and year or year only if full information is not available

 

               Cause of death: copy all the information from the death certificate, clinical notes, or any other document written by the physician. It is not enough to just write the ICD code.


NOTIFICATION

 

               Date of notification is the date when the form is filled up and signed.

 

               Notified by: name and title of person filling the form. An illegible signature is not accepted. The telephone extension may be written for ease of communication to validate information

 

               Repeat notifications are made if there is a change in the information. Just write the name and IC and fill in those parts of the form that have changed. You can write in the remarks section that this is an update

 


GENERAL TIPS

 

COMPLETE INFORMATION

               Every item must be filled.

 

               If the answer is not known write in your hand ‘not known, ‘not recorded’  ‘not relevant’ ‘not available’ etc

 

               Do not delay sending form because of unavailable information. You can send an update

 

CONSISTENCY

               Look for consistency as you fill the form

 

               Name & race

 

               Name and gender

 

               Date of birth and age

 

               IC info & gender and date of birth

 

MISTAKES

               Cross out the mistake and do not wipe it out. Both the mistake and the correction must appear

 

Corrections can be sent as updates on a new CNF but indicate so in the remarks column.
